Case Management Supervisor
General Summary Under management of the Manager of Case Management (CM), the Case Management Supervisor will lead and supervise clinical program operations including program deployment, case managers and health coaches responsible for patient care coordination in the University of Maryland Quality Care Network (UMQCN). The Supervisor provides clinical leadership for all case management programs by serving as an educator, role model, member advocate and change agent to enhance member care and staff competency. The Supervisor directs daily team operations and provides strong leadership through training, coaching, teaching, and managing assigned team members and programs. The Case Management Supervisor will evaluate patient care data to ensure that care is provided in accordance with clinical guidelines, organizational standards and identified milestones. Principal Responsibilities and Tasks The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. These are not to be construed as an exhaustive list of all job duties performed by personnel so classified. - Provides clinical guidance and supervision to all UMQCN Case Management programs, based on accepted principles of nursing, social work, gerontology, geriatrics and case management practice.
- Oversees staff and monitors department activities to evaluate the productivity and quality of programs and processes in order to identify potential improvements and to ensure maximum performance.
- Develops, leads and participates in departmental projects, workflow processes, policies and procedures in collaboration with internal and external stakeholders.
- Facilitates team meetings and develops training content for staff education and development
- Monitors program data and implements processes to impact outcomes.
- Handles escalated issues when necessary.
- Assists with providing updates and attends meetings as indicated.
- Maintains staff levels, interviewing and hiring employees and administering disciplinary actions.
- Assists in conducting employee performance reviews.
- Conducts case auditing of staff and implementation of performance improvement plans.
- Identify opportunities for business process improvements and takes lead on implementation.
- Maintains HIPAA standards and ensure confidentiality of protected health information.
- Consult with other external agencies to provide support services and resources.
- Communicate effectively with patients, physicians, and their staff on a regular basis.
